New Patient Information
First Visit
Your initial appointment, cleaning or exam, will include a consultation explaining your oral health and treatment options. Depending on the current schedule, needed treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. We will do our best to accommodate your dental needs and your schedule.
- How to Prepare for a First Appointment:
- Please assist us by providing the following information when you schedule your appointment:
- Your referral source.
- A list of your current medications.
- If you have medical or dental insurance, bring the necessary information to find out more about your plan. This will save time and allow us to help you process any claims.
- Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
- Please assist us by providing the following information when you schedule your appointment:
- X-rays
- If your prior dentist or physician has taken XRays, you may request that they forward them to our office. If possible, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
- IMPORTANT:
- A parent or guardian must accompany all patients under 18 at the consultation visit. If you have any additional questions or concerns, please give us a call at: 630-369-3120.
About Dr. Thomas F. Brown, DDS
Dr. Brown has served thousands of patients over the last 30 years. His mission is to provide exceptional and uncompromising dental care in a personal, professional, and efficient manner to all of his patients. He works diligently to be the very best oral health care professionals, with the best education, the best staff, the best service, and the most comfortable, family-oriented place for patients to be cared for.
Dr. Marie completed her undergraduate studies in Biology at the University of Illinois at Urbana-Champaign and her Doctorate in Dental Surgery at the University of Illinois at Chicago College of Dentistry. She completed part of her studies abroad in Newcastle, England and also completed multiple volunteer trips to care for underserved communities in both Jamaica and Costa Rica. Similar to Dr. Brown, she is continually learning and expanding her skills to better serve her patients and provide quality of care with most up to date technology and techniques.
